- 50 Hour Form

This is a copy of the 50 Hour Form. Before a person under age
18 can take the Illinois Road Test, proof must be provided that he or
she received an additional 50 hours of driving (with at least 10 hours
at night) with a person 21 or
older who has held their license for at least one year. Although
this form is provided by the Illinois Secretary of State, forms
created by parents will also be accepted. This form is to be
presented to the Illinois Secretary of State when taking the Road
Test.

- Proof of Eligibility Forms
- Before a person under age 18 can take the Illinois Road Test, proof
of eligibility must be provided with one of the following forms
provided by the Illinois Secretary of State. One of the
following forms are to be directed to our attention before we can
request a "Blue Slip." These forms are explained in
class and are for the convenience of students who have already taken
our course, but have yet to show proof of eligibility.

This blue form provided by the Illinois Secretary
of State is for students who have passed eight classes during the
last two semesters. Copies of Report Cards may also be used
for proof of eligibility. |
