Classroom + Six Hours Behind-The-Wheel (BTW) + Six Hours In-Car Observation
Includes 30 hours of classroom instruction over five weeks, six (6) hours of BTW instruction, and six (6) hours of in-car observation.
Classroom + Eight Hours Behind-The-Wheel (BTW)
Includes 30 hours of classroom instruction over five weeks and eight (8) hours of BTW instruction.
Six Hours BTW Only
Includes six (6) hours of BTW instruction and six (6) hours of in-car observation.
Eight Hours BTW Only
Includes eight (8) hours of BTW instruction.
Includes 30 hours of classroom instruction.

This is a copy of the 50 Hour Form. Before a person under age 18 can take the Illinois Road Test, proof must be provided that he or she received an additional 50 hours of driving (with at least 10 hours at night) with a person 21 or older who has held their license for at least one year. Although this form is provided by the Illinois Secretary of State, forms created by parents will also be accepted. This form is to be presented to the Illinois Secretary of State when taking the Road Test.
Proof of Eligibility Forms
Before a person under age 18 can take the Illinois Road Test, proof of eligibility must be provided with one of the following forms provided by the Illinois Secretary of State. One of the following forms are to be directed to our attention before we can request a “Blue Slip.” These forms are explained in class and are for the convenience of students who have already taken our course, but have yet to show proof of eligibility.
This blue form provided by the Illinois Secretary of State is for students who have passed eight classes during the last two semesters. Copies of Report Cards may also be used for proof of eligibility.
This pink waiver form provided by the Illinois Secretary of State is for persons who have NOT passed eight classes during the last two semesters.
This gold form provided by the Illinois Secretary of State is for students who are home schooled.
