Although this form is provided by the Illinois Secretary of State, forms created by parents will also be accepted. This form is to be presented to the Illinois Secretary of State when taking the Road Test.

Proof of Eligibility Forms
Before a person under age 18 can take the Illinois Road Test, proof of eligibility must be provided with one of the following forms provided by the Illinois Secretary of State. One of the following forms are to be directed to our attention before we can request a “Letter of Completion.” These forms are explained in class and are for the convenience of students who have already taken our course, but have yet to show proof of eligibility.

This blue form provided by the Illinois Secretary of State is for students who have passed eight classes during the last two semesters. Copies of Report Cards may also be used for proof of eligibility.

This pink waiver form provided by the Illinois Secretary of State is for persons who have NOT passed eight classes during the last two semesters.

This gold form provided by the Illinois Secretary of State is for students who are home schooled.

 Scheduling Behind-The-Wheel (BTW)

Behind-The-Wheel (BTW) lessons continue in accordance with Secretary of State guidelines.  When scheduling a BTW lesson, students will select the pickup location.  Please enter the full address.  If the pickup location is more than 15 minutes from the classroom location, the instructor may ask the student to meet at a half way point.  All students with permits are encouraged to log in and schedule Behind-The-Wheel lessons.

Road Test Information:
You can take the road test with us if you received an A or B (80% or above) in the classroom (even at another school).  We are state-certified to administer the road test.  Please call 630-879-7700 to schedule an appointment for a road test and skip the line at the Secretary of State.
